Qualifications in becoming a Direct Care Worker:
Must be 18 years of age

Driver’s License / Identification Card
Social Security Card
First Aid Certification (NO web-based training)
CPR Certification (NO web-based training)
Fire Safety Certification
Food Handlers Permit (blue card)
DPS Fingerprint Clearance Card
10-year Navajo Nation Background Check
DCW Training Certification: Level 1 - Fundamentals
Level 2 - Aging & Physical Disabilities
Level 2 - Developmental Disabilities
(We do in-house DCW training, if needed)
****All certifications must be current and non-expired.
Qualifications in becoming a Non-Emergency Medical Transportation Driver:
Must be 26 years of age
Valid AZ Drivers License
DPS Fingerprint Clearance Card
Pre-Employment Drug Test
Department of Motor Vehicle 5-year record
First Aid / CPR Course Training
Navajo Nation Background Check
AZ State / Federal Background Check
AZ Department of Transportation Physical Exam
Defensive Driving Course Training
HIPAA Training
